I-STEM Information Upload

Please use the secure form below to upload your personal and payment information to CIESC and the IDOE in order to receive your payment from the I-STEM program.

Please note: your submission is not saved in our system! Your W-9 and ACH forms will be submitted to the CIESC finance office directly. We will not be able to retrieve or edit your submissions. You must contact the CIESC Business Manager, David Stowers, at dstowers@ciesc.org if you need to make any changes.

IDOE Forms

These forms will be submitted to the IDOE to confirm your enrollment into the I-STEM program and for payment verification and auditing purposes. These documents will NOT be stored on our servers. Only the CIESC Business Manager will have access to these documents in order to send them directly to the IDOE.
